Please use our PDF form when submitting Bank Notes to Canadian Currency Grading Service for grading.

By signing the form you agree to the terms and conditions as described in CCGS Grading Legalities.

Please print both pages of the form!

Please do the following:
1.) Fill in your name and contact information.
2.) Complete all descriptive information regarding the note(s).
3.) Calculate Total Grading and Shipping/Insurance rates.
4.) Calculate Grand total with all fees and insert total on the form.
5.) Sign and date agreement (front of form, bottom left).

Full Payment Must Accompany The Order. Make checks payable to Canadian Currency Grading Service, Inc., or (CCGS)
